Supportive Living Program Expands Enrollment for Dementia Care

The Department of Healthcare and Family Services (HFS) announced on August 24, 2016 that they are expanding enrollment for the Supportive Living Program for dementia care settings for experienced healthcare providers. The application process will require interested parties to demonstrate experience in working with individuals in a dementia care setting.

Dementia care patients are those who have a mental inability that is severe enough to prevent an individual from being able to perform normal activities of daily living. Diagnosis that are commonly found in dementia care patients include Alzheimer’s, Parkinson’s, Huntington’s disease, or mixed dementia.

Below are the per diem rates for dementia care settings by geographic area:

Chicago  $110.20
South Suburban $101.23
Northwest  $96.84
Central $93.67
West Central $85.03
St. Louis   $93.64
South  $81.50

Certified Supporting Living providers will be allowed to submit applications beginning September 1, 2016 through December 30, 2016.  Providers should be aware that the State is not excepting applications for Kane, Champaign, Effingham, or Macon County unless it is to expand an existing Supportive Living dementia care setting. Other healthcare providers will be able to submit applications to HFS from December 1, 2016 through March 31, 2017.
